Is there some sort of Menu Template with comments?

I would like to learn how to make a menu plugin by myself. I want to learn from experience, but the code, to me, makes little to no sense... so, that being said, I'd like it if someone could provide a sort of... template menu? A simple, quick plugin, with a simple design and heavy comments that explain in simple terms how each part contributes to the overall scene, that way I have something that's Easy to Learn From.
